| Objective To identify important research and trends over the past two decades in the development of brain-computer interface (BCI) technology as it applies to post-stroke motor rehabilitation. Methods English literature related to BCI technology and post-stroke motor recovery published between 2006 and 2025 was reviewed. CiteSpace software was employed to conduct a visual analysis of publication trends, national and institutional collaboration networks, author clusters, keyword co-occurrence and clustering, and burst terms. Results A total of 498 articles were included. The annual publication volume grew rapidly over the period, particularly after 2014. The United States ranked first in both publication output and collaborative influence. China ranked high in publication volume but requires strengthened international cooperation. Major high-productivity institutions included the University of Tübingen and its affiliated hospitals, the Agency for Science, Technology and Research (A*STAR) in Singapore, and Fudan University in China. Prominent authors (e.g. Niels Birbaumer) have formed stable research teams. High-frequency keywords included "brain-computer interface", "motor rehabilitation", "stroke" and "motor imagery". Cluster analysis revealed themes such as the optimization of motor imagery in BCI training and the application of BCI integrated with other rehabilitation technologies (e.g. virtual reality, electrical stimulation or robotics). This reflected a shift from the exploration of mechanisms to clinical validation, and an expansion from single-technology approaches to multi-technology integration. Burst term analysis indicated that "lower-limb rehabilitation" and "functional near-infrared spectroscopy" have been active topics in recent years. Conclusions BCI research in stroke rehabilitation has expanded substantially, evolving toward a multimodal "BCI+X" paradigm integrating robotics, stimulation and virtual reality. Future directions can be expected to include lower-limb applications, multimodal systems, and studies of neuroplasticity′s mechanisms. |
