|  目的 观察基于脑卒中专用生活质量量表(SS-QOL)设计的虚拟现实训练(VR)对脑卒中后上肢功能障碍患者生活质量和上肢功能的影响。 方法 本研究采用前瞻性随机对照试验设计,共纳入脑卒中偏瘫患者70例,按随机数字表法分为试验组和对照组,每组患者35例。对照组接受连续4周的常规康复训练,试验组在此基础上增加基于SS-QOL的VR训练,每日1次,每次30 min,每周治疗5 d,连续治疗4周。干预前和干预4周后(干预后)采用脑卒中专用生活质量量表(SS-QOL)、上肢运动功能评定量表(FMA-UE)、香港版偏瘫上肢功能测试(FTHUE-HK)及Barthel指数(BI)分别评估2组患者的生活质量、上肢运动功能和日常生活活动能力。 结果 干预后,2组患者SS-QOL量表中的上肢功能、情绪状态、社交参与评分和总分较组内干预前均显著改善(P<0.05),且试验组干预后SS-QOL量表中的上肢功能、情绪状态、社交参与评分和总分分别为(26.5±3.0)分、(35.4±4.3)分、(20.3±2.1)分、(192.7±13.2)分,较对照组干预后均显著改善,差异均有统计学意义(P<0.01)。干预后,2组患者的FMA-UE、FTHUE-HK及Barthel指数评分较组内干预前均显著改善(P<0.05),且试验组的FMA-UE、FTHUE-HK和Barthel指数评分亦显著优于对照组干预后(P<0.05)。 结论 基于SS-QOL的VR训练可更为显著地改善脑卒中后上肢功能障碍患者的生活质量和上肢功能。 |

| Objective To document the effects of virtual reality (VR) training based on the Stroke-specific Quality of Life Scale (SS-QOL) on the life quality and upper limb functioning of stroke survivors with upper limb impairment. Methods This was a prospective randomized and controlled trial. Seventy hemiplegic stroke survivors were randomly divided into an experimental group and a control group, each of 35. Both groups received routine rehabilitation training for 4 consecutive weeks. The experimental group additionally underwent daily 30-minute sessions of VR training based on the SS-QOL. Before and after the intervention, the SS-QOL, the Fugl-Meyer Upper Extremity Assessment (FMA-UE), the Hong Kong version of the Functional Test for the Hemiplegic Upper Extremity (FTHUE-HK), and the Barthel Index (BI) were used to evaluate quality of life, upper limb motor function, and ability in the activities of daily living. Results After the intervention, both groups′ upper limb function, emotional status and social participation scores, and their total scores on the SS-QOL had improved significantly, on average. The experimental group′s averages were then (26.5±3.0), (35.4±4.3), (20.3±2.1) and (192.7±13.2), respectively, all significantly superior to the control group′s averages. Both groups′ average FMA-UE, FTHUE-HK and BI scores had also improved significantly, with the improvements in the experimental group again significantly greater than those in the control group. Conclusions Virtual reality training based on the SS-QOL can significantly improve quality of life and upper limb function for persons with upper limb dysfunction after a stroke. |
