|  目的 探讨电针联合康复训练对脑缺血再灌注损伤(CIRI)大鼠脑组织损伤及神经调节素1(NRG1)/酪氨酸激酶受体4(ErbB4)信号通路的影响。 方法 将108只实验大鼠随机分为对照组、CIRI组、跑台训练组、电针组、联合干预组及联合干预+NRG/ErbB通路抑制剂(AG825)组,每组18只大鼠。对照组大鼠仅分离暴露左颈总动脉、颈内动脉、颈外动脉,不进行线栓插入操作,其他各组大鼠均制成CIRI动物模型。于制模成功24 h后跑台训练组大鼠给予跑台(坡度为0°)训练,电针组大鼠给予电针干预,联合干预组大鼠先给予电针治疗,再进行跑台训练;联合干预+AG825组大鼠先通过尾静脉注射AG825,随后依次进行电针及跑台训练。经持续干预14 d后检测各组大鼠神经功能评分及脑梗死面积比值;采用苏木精/伊红(HE)染色观察海马组织病理学变化;采用ELISA法检测海马组织肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-6、IL-10水平;采用原位末端标记法(TUNEL)检测海马组织神经元凋亡情况;采用Western blot法检测海马组织p53、天冬氨酸特异性半胱氨酸蛋白酶-3(Caspase-3)、NRG1及p-ErbB4蛋白表达。 结果 与CIRI组比较,跑台训练组、电针组、联合干预组神经元间隙变小,核固缩及炎性细胞浸润程度减轻,神经功能评分、脑梗死面积比值、海马组织TNF-α、IL-6水平、神经元凋亡率及p53、Caspase-3蛋白表达均明显降低,海马组织IL-10水平及NRG1、p-ErbB4蛋白表达均明显升高,且以联合干预组上述指标的变化幅度尤为显著(P<0.05);AG825能减弱电针联合康复训练对CIRI大鼠脑损伤的改善作用。 结论 电针联合康复训练可抑制CIRI大鼠神经炎症反应及神经元凋亡,减轻脑损伤,其作用机制可能与激活NRG1/ErbB4通路有关。 |

| Objective To document any effects on brain tissue injury resulting from cerebral ischemia and reperfusion of supplementing conventional rehabilitation protocols with electroacupuncture. Methods A total of 108 rats were randomly divided into a control group, a cerebral ischemia and reperfusion injury (CIRI) group, a rehabilitation training group, an electroacupuncture group, a combination group, and a combination + neuregulin 1 (NRG1)/receptor tyrosine protein kinase 4 (ErbB4) pathway inhibitor (AG825) group, each of 18. The control group was only isolated and had its carotid artery exposed without suture insertion. The CIRI model was induced in the rates of the other groups. Twenty-four hours after successful modeling, 14 successive days of the respective treatments began. Neurological functioning and the proportion of cerebral infarct area were observed. Hematoxylin-eosin staining was used to detect hippocampus pathology. Enzyme-linked immunosorbent assays were employed to measure tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), and IL-10 in the hippocampal tissue. Terminal deoxynucleotidyl transferase dUTP nick end labeling staining was conducted to measure neuron apoptosis rates, and western blotting was used to detect p53, caspase-3, NRG1, and p-ErbB4 proteins in the hippocampal tissues. Results All the other groups except the control group showed a significant decrease in neuron gaps, nuclear condensation, inflammatory cell infiltration, their average neurological function scores, the proportion of cerebral infarct area, TNF-α and IL-6 levels, neuron apoptosis rate, and p53 and Caspase-3 proteins in their hippocampal tissue compared with the CIRI group, but they showed a significant increase in the IL-10 level, and NRG1 and p-ErbB4 proteins in their hippocampal tissue. The most conspicuous changes were in the combination group. AG825 weakened the effect of electroacupuncture combined with rehabilitation training. Conclusions Supplementing rehabilitation training with electroacupuncture can inhibit neuroinflammation and neuron apoptosis and alleviate brain damage, at least in rats. Its mechanism may be related to activation of the NRG1/ErbB4 pathway. |
