      目的 探讨呼吸手法干预联合吞咽干预对中晚期早产儿经口喂养表现的改善作用。 方法 采用随机数字表法将69例存在经口喂养障碍的中晚期早产儿分为观察组(34例)及对照组(35例)。2组患儿均给予口腔运动刺激、非营养性吸吮、体位管理、环境管理等干预,观察组患儿在此基础上辅以呼吸手法干预。于干预前、干预1周后对比2组患儿进乳速率(RT)、喂养效率(PRO)、早产儿经口喂养评估量表(PIOFRA)评分、进乳过程中经皮血氧饱和度及体质量改善情况。 结果 干预后观察组及对照组患儿PIOFRA评分[分别为(28.56±2.67)分、(26.31±2.19)分]、进乳速率[分别为(2.60±0.16)ml/min、(2.03±0.35)ml/min]、喂养效率[分别为(48.77±7.65)%、(43.18±5.13)%]、血氧饱和度最低值[分别为(78.52±5.26)%、(75.69±4.90)%]均较干预前明显增加(P<0.05),血氧饱和度<90%事件发生次数[分别为(2.62±1.9)次、(4.29±1.15)次]均较干预前明显减少(P<0.05);并且上述指标均以观察组患儿的改善幅度更显著,与同期对照组间差异均具有统计学意义(P<0.05)。干预前、后2组患儿体质量组间差异均无统计学意义(P>0.05)。 结论 呼吸手法干预联合吞咽干预能有效改善中晚期早产儿经口喂养表现,为临床优化早产儿喂养管理提供了新策略。
英文摘要:
      Objective To document any effect of supplementing swallowing training with breathing manipulation intervention on the oral feeding ability of mid- and late-term preterm infants. Methods A total of 69 mid- and late-term preterm infants with disordered oral feeding were randomly divided into an observation group (34 cases) and a control group (35 cases). Both groups received a week of oral motor stimulation, non-nutritive sucking, position management and environment management, but the observation group was additionally provided with breathing manipulation intervention. Before and after the intervention, both groups were evaluated using the Preterm Infant Oral Feeding Readiness assessment (PIOFRA), and in terms of milk intake rate (RT) and feeding efficiency (PRO). Their oxygen saturation during feeding and weight improvement were also recorded. Results After the interventions, both groups showed significant increases in their average PIOFRA scores [observation group: (28.56±2.67) vs. control group: (26.31±2.19)], their average milk intake rate [observation group: (2.60±0.16)ml/min vs. control group: (2.03±0.35)ml/min], their feeding efficiency [observation group: (48.77±7.65)% vs. control group: (43.18±5.13)%], and their average minimum oxygen saturation [observation group: (78.52±5.26)% vs. control group: (75.69±4.90)%]. The improvements were all significantly greater in the observation group than among the control group. The number of instances of oxygen saturation <90% also decreased more in the observation group (to 2.62±1.9 vs. (4.29±1.15) (P≤0.05). There were, however, no significant differences in weight between the two groups. Conclusions Breathing manipulation intervention combined with swallowing intervention can effectively improve the oral feeding performance of mid- and late-term preterm infants, providing a new clinical technique for optimizing their feeding.
