| 目的 观察太极拳“云手”训练对中老年肩周炎患者功能康复的影响。 方法 采用随机数字表法将100例肩周炎患者分为对照组和试验组,每组50例。2组患者均给予常规治疗,在此基础上,对照组增加基础动作训练,试验组增加太极拳“云手”训练。治疗前及治疗2、4、8周后,比较2组患者的肩关节活动度,并采用Constant-Murley肩关节评分量表(CMS)、数字评分法(NRS)、焦虑自评量表(SAS)及匹兹堡睡眠质量指数(PSQI)分别评估2组患者的肩关节功能、疼痛程度、焦虑水平及睡眠情况。治疗8周后,对2组患者的疗效进行比较。 结果 与组内治疗前比较,2组患者治疗2、4、8周后的肩关节活动度增加(P<0.05),CMS评分增加(P<0.05),NRS、SAS、PSQI评分下降(P<0.05)。与对照组比较,试验组治疗2、4、8周后肩关节的前屈角度、外展角度、内旋角度、外旋角度较大(P<0.05),CMS评分较高(P<0.05),NRS、SAS、PSQI评分较低(P<0.05)。治疗8周后,试验组的治疗总有效率(98%)显著高于对照组(84%),差异有统计学意义(P<0.05)。 结论 在常规治疗基础上,联合太极拳“云手”训练可显著改善肩周炎患者的肩关节功能,减轻疼痛及焦虑症状,提高睡眠质量。 |

| Objective To observe the effect of practicing tai chi′s cloud hands maneuver in the functional rehabilitation in middle-aged and elderly persons with frozen shoulder. Methods One hundred adults with frozen shoulder were randomly divided into a control group and an experimental group, each of 50. In addition to conventional treatment, the control group received basic exercise training, while the experimental group was trained to perform tai chi′s cloud hands maneuver. Before and after 2, 4 and 8 weeks of training, shoulder range of motion was compared between the two groups. Constant-Murley Shoulder Outcome scores (CMSs) were assigned, a numeric rating scale (NRS) was used, along with the Self-Rating Anxiety Scale (SAS), and the Pittsburgh Sleep Quality Index (PSQI) to assess shoulder function, pain intensity, anxiety, and sleep quality. After 8 weeks of the treatments, the therapeutic effects were compared between the two groups. Results Both groups showed increased shoulder range of motion at 2, 4 and 8 weeks. Both groups′ average CMS scores had improved significantly, and their average NRS, SAS and PSQI scores had decreased significantly. At each time point the experimental group exhibited greater forward flexion, abduction, internal rotation, and external rotation, on average, as well as significantly higher CMS scores and lower NRS, SAS, and PSQI scores. After 8 weeks the experimental group′s total effectiveness rate was significantly higher than that of the control group. Conclusion Adding cloud hands training to conventional treatment can significantly improve shoulder joint function, relieve pain and anxiety, and enhance sleep quality in cases of frozen shoulder. |
