      目的 探讨Physio Touch淋巴回流促进治疗对全膝关节置换术(TKA)后患者肢体肿胀程度及血液炎性指标的影响。 方法 采用随机数字表法将42例TKA患者分为对照组和试验组,每组21例。研究期间,对照组有1例患者脱落,最终对照组与试验组分别纳入20例、21例。2组均给予TKA术后常规康复治疗,试验组在此基础上增加Physio Touch淋巴回流促进治疗。术前、术后7 d,比较2组患者的膝关节周径、膝部切口皮温、疼痛视觉模拟评分法(VAS)评分、膝关节屈伸活动度、实验室炎性指标(白细胞计数、C反应蛋白)变化,并记录并发症。 结果 术后7 d,2组患者的膝关节周径、膝部切口皮温、膝关节屈伸活动度、C反应蛋白水平均较术前有所增加(P<0.05)。与对照组比较,试验组术后7 d髌骨中点上10 cm处的膝关节周径[(34.00±2.30)cm]较小,膝部切口皮温[(36.85±0.35)℃]较低,膝关节屈伸活动度[(89.43±5.60)°]较大,白细胞计数[(5.33±0.93)×109/L]和C反应蛋白水平[(12.96±4.28)]较低(P<0.05)。2组患者术前及术后7 d均未见深静脉血栓,术后7 d手术切口未见出血、感染、延迟愈合、明显皮肤反应或过敏反应。 结论 TKA术后应用Physio Touch淋巴回流促进治疗能有效减轻患者的肢体肿胀程度,改善膝关节活动度,降低炎症反应,值得临床应用、推广。
英文摘要:
      Objective To document any effect of physio touch therapy on limb swelling and inflammatory indicators in treating patients after total knee arthroplasty (TKA). Methods Forty-one TKA patients were allocated to either a control group of 20 or an experimental group of 21. All received routine rehabilitation, but the experimental group additionally underwent physio touch therapy. Before the surgery and 7 days afterward, knee circumference, skin temperature around the incision, visual analog scale pain ratings, knee flexion and extension range of motion, white blood cell count and C-reactive protein levels were compared between the two groups. Complications were also recorded. Results Compared with the pre-operative values, both groups showed significant increases in knee circumference, skin temperature around the incision, knee flexion and extension range of motion, and C-reactive protein levels on day 7. Compared with the control group, the experimental group then had, on average, a smaller knee circumference at 10cm above the midpoint of the patella, lower skin temperature, a greater range of motion, and lower white blood cell counts and C-reactive protein levels. No deep vein thrombosis, bleeding, infection, delayed healing, significant skin reactions, or allergic reactions were observed. Conclusions Physio touch therapy after TKA can effectively reduce the swelling, improve knee joint range of motion, and reduce inflammatory responses. It is worthy of clinical application and promotion.
